This fragmented approach can complicate the healthcare journey, limit the impact of therapeutic advances and place increasing pressure on healthcare systems.

The CKM Collaborative exists to challenge this model – bringing together clinicians, educators and healthcare partners to drive a more integrated, person-centred approach to care.
